Primary Care Physician Role Overview: Join our team in Wailuku, Maui, and provide high-quality, veteran-centered primary care while enjoying an exceptional work-life balance in one of Hawaii’s most beautiful communities.
Role Overview: The Primary Care Physician delivers comprehensive outpatient care using the Patient Aligned Care Team (PACT) model. Responsibilities include evaluating and treating adult patients, managing chronic conditions, coordinating specialty services, and contributing to quality improvement and team-based care.
Key Responsibilities:
•Provide preventive care, chronic disease management, patient education, and shared decision-making.
•Conduct exams, order and interpret diagnostics, develop treatment plans, and provide follow-up care.
•Participate in quality improvement initiatives and multidisciplinary team activities.
•Serve as a clinical leader and mentor for trainees and colleagues.
•Coordinate care across specialties to ensure seamless, veteran-focused services.
